The Ancient Rivers Family Health Team (FHT) is a group of interprofessional healthcare providers associated with the Ancient Rivers Family Health Organization, a group of 17 family doctors in Renfrew and Calabogie. We have a combined roster of 14,000 patients on one shared electronic medical record system (Telus Practice Solutions).
We are recruiting for a Nurse Practitioner (NP). This can be a full-time or part-time position.
Working within our interdisciplinary team, the Nurse Practitioner provides high-quality, full scope professional care, including primary care and participates in the development, implementation, monitoring, and evaluation of the FHT’s programs and services. The NP will have the opportunity to attach patients and work within a ‘most responsible provider’ model of care as part of their role.
Main Responsibilities:
- Provide comprehensive, evidence-based care in alignment with the College of Nurses of Ontario (CNO) Practice Standards and the competencies outlined by the Canadian Nurses Association. This includes the authority to diagnose, order and interpret diagnostic tests, prescribe medications, and perform medical procedures.
- Demonstrate advanced clinical judgment and a high level of autonomy in managing acute and chronic conditions, as well as promoting health and wellness.
- Attach and provide care for a roster of up to 800 patients per 1.0 FTE as the most responsible provider, ensuring continuity and quality of care.
- Support the care of physician-rostered patients as part of the collaborative clinical team.
- Design and deliver care programs for unattached patients, such as post-emergency department discharge follow-up, urgent care clinics, and cancer screening clinics.
- Lead and participate in health promotion and disease prevention initiatives to support overall community wellness.
- Effectively utilize the electronic medical records system for documentation, assessment, care planning, and communication within the care team.

- Certificate of Registration in the Extended Class with the College of Nurses of Ontario (RN-EC), and eligible to practice in Ontario.
- Current membership with RNAO and proof of liability insurance.
- Experience in a primary care setting is preferred.
- Strong health assessment, diagnostic, and clinical judgment skills for managing a broad range of health concerns.
- Strong interpersonal, verbal, and written communication skills.
- The ability to effectively engage with patients, families, and interdisciplinary team members.
- Proficient in evidence-based primary care, with a focus on health promotion, prevention, and risk reduction.
- Experience in patient education and chronic disease management.
- Comfortable using Microsoft Office and EMRs.
- Experience with Telus PS Suite is an asset.
- Valid Ontario driver’s license and access to a vehicle for regional travel.
