The Ancient Rivers Family Health Team (FHT) is a group of interprofessional healthcare providers associated with the Ancient Rivers Family Health Organization, a group of 17 family doctors in Renfrew and Calabogie. We have a combined roster of 14,000 patients on one shared electronic medical record system (Telus Practice Solutions).
We are recruiting for a Registered Dietician. This is a temporary full-time or part-time position until March 31, 2027
The Registered Dietitian (RD) will work closely with our primary care providers and integrated healthcare providers as part of a dynamic interdisciplinary healthcare team. The RD provides medical nutritional therapy to the patients of the FHT and participates in the development, implementation, monitoring, and evaluation of the FHT’s programs and services.
Main Responsibilities:
- Provide direct patient care, with a focus on chronic disease management
- Conduct assessments and provide consultations, in person or virtually, to patients referred to the dietitian program
- Develop and deliver education to patients, other team members and community partners pertaining to nutrition and health
- Provide education to patients in a way that best meets their individual learning abilities and that optimizes patient encounters
- Incorporate principles of patient self-management into every patient visit
- Collaborate with physicians, nurse practitioners and interdisciplinary healthcare professionals to make decisions regarding the delivery of services to patients, and to facilitate patient-centered care
- Communicate effectively with physicians and nurse practitioners to provide information pertaining to the patient’s care plan
- Utilize the electronic medical records system to record and review information pertaining to patients that can be used for patient assessments and consultations
- Collaborate with administrative assistants as needed to ensure optimal scheduling for patients, and to provide input into the scheduling process
- Lead, participate and provide input into a variety of team meetings, projects and committees
- Assess and make recommendations regarding nutrition services to ensure it aligns with best-practices and the FHT’s strategic direction
- Connect and build relationships with community partners
- Input statistical information through an established process that is aligned with the ministry's reporting requirements
- Update and maintain nutritional resources to ensure up-to-date and best-practice information is provided to patients

- Bachelor’s degree in nutrition and dietetics or similar
- Graduate of an accredited dietetic internship program
- Registered and in good standing with the College of Dietitians of Ontario
- Membership with Dietitians of Canada is an asset
- Certified Diabetes Educator (CDE) certification is an asset
- Three years of related experience in a community or primary healthcare setting
- Experience in program development and evaluation is an asset
- Demonstrated ability to communicate effectively with patients, families, and members of an interdisciplinary medical team
- Excellent written and verbal communication skills
- Intermediate knowledge of Microsoft Office Suite and EMR (Telus Practice Solutions)
- Experience in patient education and chronic disease management
- Demonstrated ability to work well independently and in a team environment
- Current Ontario driver’s license and access to a vehicle for travel within the region
