Columbus House seeks to improve the quality of life for children, youth, and families in Renfrew County by providing programs which focus on the well-being and education.  We offer these supports in an atmosphere that celebrates diversity, cultural sensitivity, inclusivity, and family involvement.

Columbus House operates two sites for The Guest House which supports youth in crisis, unhoused and risk for being unhoused, and/or youth with more complex needs waiting for referrals for addiction and mental health supports.

Columbus House and Youth Wellness Hus Ontario (YWHO) are recruiting for a creative and passionate individual to join our team as an Intensive Case Worker. This is a full-time, three-year contract position which primarily includes day shifts, however, there will be flexibility required for some evenings or weekends.

Responsibilities:

• Be the case manager for youth in the Guest House beds
• Contribute to the continuation of the development of Guest House beds including preparing policies procedures and protocols
• Develop and update memorandums of understanding with other service providers
• Developing a pool of staff and/or staffing model for Guest House in Pembroke
• Provide informal counselling and make referrals 
• Work as part of the youth wellness hub integrated team 
• Develop and maintain partnerships with community agencies dealing with youth

• SSW, CYW or a diploma or degree in a related field; students in their last year of study are welcome to apply, and equivalent work experience will be considered
• Excellent communication skills (verbal and written)
• Ability to work independently and be effective in crisis situations
• Must have and maintain a valid Ontario driver's license
• Experience working with youth or at-risk populations
• Bilingualism is an asset
• A clear vulnerable sector screening check
• A good understanding of social services and other agencies working with youth in the County of Renfrew

The Intensive Case Manager will be integrated within the Renfrew County Youth Wellness Hub team and will be required to travel throughout the county.  Mileage is reimbursed under CRA recommendations if your own vehicle is used.  A company vehicle will be provided when available. This position offers a generous benefits package.
