ConnectWell Community Health is a fully accredited health centre providing primary health care, health promotion programs, and illness prevention services. At ConnectWell, the health and well-being of every individual, every family, and every community matters! Our staff includes family doctors, nurse practitioners and nurses, a social worker, a respiratory therapist, a chiropodist, a health promoter, and administrative support. Together, we provide a wide range of primary care and health promotion programs and services to area residents.

ConnectWell Community Health is recruiting two Nurse Practitioners to join our interdisciplinary team. One position is part-time and one position is full-time.

The full-time position will begin in April 2024 and is for 35 hours per week. You will be providing primary care to clients at the community health centre in Beachburg, Cobden and Eganville.  This position also provides primary care to youth ages 12 to 25 at the youth wellness hub in Pembroke for 7 hours per week.

The part-time position will begin July 2024 and is for 28 hours per week. You will be providing primary care to clients at the community health centre in Beachburg, Cobden and Eganville.

Qualifications
  • RN (EC) license, Nurse Practitioner Primary Health Care certificate
  • Current and unrestricted registration as an RN (EC) with the College of Nurses of Ontario
  • Three years of experience in the nurse practitioner role in a primary care or community setting is preferred
  • Demonstrated ability to work effectively in an interdisciplinary team
  • Strong clinical skills
  • Proven commitment to and knowledge of community-based health care, preferably in a rural community and with disadvantaged population groups
  • Experience in the development and delivery of health education and promotion programs
  • Ability to work flexible hours, if required
  • Valid driver’s license and use of a vehicle
  • Attention to detail
  • Ability to prioritize
  • Time management skills
  • Excellent interpersonal, oral, and written communication skills
