Algonquins of Pikwakanagan First Nation (AoPFN)is a proud and progressive Algonquin community. Pikwakanagan is situated on the shores of Golden Lake and the Bonnechere River in Renfrew County. Located off of Highway 60, our community is 90 minutes west of Ottawa and 90 minutes hours east of Algonquin Park.

We are currently recruiting for a Home and Community Care Coordinator. This is a full-time position.

The Home and Community Care Coordinator functions as a member of the health team to coordinate components of the home care services. Additional responsibilities:

  • Develop and maintain a good working relationship with all government agencies, indigenous agencies and business associates.
  • Implement the essential service elements as identified in the service delivery plan.
  • Provides care management including assessments and reassessments; develop care plans based on the assessment, using the appropriate assessment tools.
  • Coordinate accessibility through referrals to the remaining service elements.
  • Maintain existing levels of service and access to remaining service elements.
  • Participate or coordinate case conferences: incl. hospital discharge plan.
  • Provide palliative care including or on-call if required.
  • Provide on-call nursing services or home visits to members as required.
  • Complete and keep accurate clinical records.
  • Oversee the loan cupboard program and home maintenance.
  • Function as a member of the health team.
Qualifications
  • Bachelor of science, bachelor of nursing or registered nurse.
  • Current registration with the College of Nursing and in good standing.
  • Current membership with the Registered Nurses Association of Ontario.
  • Carry current malpractice insurance.
  • Experience working with a First Nations community or indigenous population is considered an asset.
  • Certified in CPR and First Aid.
  • Minimum of two years of nursing experience in an Aboriginal community.

Conditions of Employment:

  • Proof of registration with RNAO.
  • Proof of membership with CNO.
  • Provide an acceptable criminal records check and vulnerable sector check.
  • Valid First Aid, CPR, AED, health and safety worker certification, and WHMIS.
  • Participate and complete training (cultural awareness and sensitivity training).

Interviews: Interviews will be conducted in-person or virtually by Teams Meeting. AOPFN has the right to shortlist for interview and assessment purposes of the five most qualified persons and typically establishes an eligibility list.
